  • Medication reviews for Nightingale patients in Indiana, Illinois, Minnesota, California, Nevada and Texas to identify potential irregularities. These irregularities are communicated to clinicians and physicians when appropriate. Findings may include such items as:
    1. Duplicate therapy
    2. Potential side effects which may interfere with care
    3. Potential drug interactions that may occur
    4. Request lab orders to help manage medication therapy
    5. Identify patients who would benefit from HomMed telemonitor or Med Partner

    Role of a Pharmacist at Nightingale
  • Communicate directly with Telemedicine nurse to help manage medication related Problems when needed.

  • Track average medication use for Nightingale Home Healthcare patients.

  • Manage and organize annual flu vaccine campaign:
    1. Develop policies and procedures for handling, documenting, storage and supplies
    2. In-service appropriate staff re: vaccine administration, documentation and reaction preparedness

  • Certified Immunizing Pharmacist and therefore administer vaccines.

  • Contributing Editor for Radius. Write timely articles relating to medication use for quarterly publication.

  • Educate staff with timely, useful information regarding medications.

  • Maintain clinician resource library. Information available includes: patient aids in helping to manage Diabetes, Food-drug interactions listings, References regarding dietary supplements, wound treatment and Medications which interfere with wound healing.

  • Provide "brown bag" medication information sessions; speak to senior living facilities about medications, uses, generics, OTC, food - drug interactions, etc. as needed.

  • Contribute educational studies for Nightingale University.
